Provider Information
Providers of service are a vital part of the new service system planning in Northeastern Wisconsin. Northeast Wisconsin Family Care is responsible for developing an adequate network of providers. The growth in consumers through serving wait lists and an entitlement to the service will significantly increase the need for providers in Northeastern Wisconsin.
As part of the planning, Northeast Wisconsin Family Care will prioritize working with providers to identify gaps in services and to conduct resource and capacity development.
Specific forms or other communication activities will be scheduled with providers as the process moves closer to implementation. Providers are encouraged to follow planning activities and announcement of upcoming meetings and forums on this website.
